Extension du spectre de l’autisme

Abstract : Objectives : Propose a clinical set under the title of autism spectrum disorders, and differentiate it from infantile psychosis. Methods : From a state of art and a number of clinical cases, we suggest to specify the autism spectrum, and we try to differentiate autism from infantile schizophrenia. Lacan in the Seminar XI talked about an “edge structure” to account for the source of the drive. This structure circulates from a body orifice, the erogenous zone, circles around the object without reaching it, and provides satisfaction by returning to the erogenous zone. The impulse originates therefore from a gaping edge, a boundary that establishes the circuit identifying the port and the object. However, if we consider the absence or inability of the drive circuit, then it is necessary to think of another term, another edge to contain enjoyment. According to Eric Laurent's formula, in autism, enjoyment returns to an edge and provides an essential element of the autistic structure. Results : The core of the spectrum lies within the transitions from Kanner's to Asperger's syndrome, but the spectrum ranges on a much wider scope. Autistic defence, characterised by the construction of an edge, develops from the isolating to the dynamic edge. In the case of high level autism, it may even finally erase the edge. Discussion : When an edge has not been constructed, autism appears to be very difficult to distinguish from schizophrenia. However, when the edge has been erased, autism can become almost undistinguishable and never diagnosed as such. Conclusion : A spectrum can be highlighted with regard to autism when relying upon Kanner and Asperger‘s remarkable clinical insight and, consequently, when avoiding the dissolution of autism in the hotchpotch of pervasive developpemental disorders.
